To submit this form, please email your completed document to submissions@ahainstructors.org. For fax submissions, send to (555) 123-4567. You may also submit through the online form available on the AHA Instructor Network website. Ensure all necessary fields are filled before submission to avoid processing delays.

For 2024, ensure all course completion cards are printed by March 1 for timely trainings. In 2025, renewals will begin on January 15, so keep this in mind when issuing cards.

The purpose of this form is to certify individuals who have completed the AHA Heartsaver First Aid, CPR AED Program.
